A premier service for families and professionals, Elder Care Guides’ Care Managers are certified local experts who work with you to evaluate your situation, coordinate in-home services, and provide ongoing support around the challenges of aging and disability. We serve older adults with physical and cognitive disabilities, as well as adults with chronic mental illness, and those with developmental and intellectual disabilities.
Our Care Managers conduct a clinical assessment, develop a personalized plan, and use our experience, knowledge, and familiarity with community resources to effectively manage the care that is provided. As advocates, we provide clarity and peace of mind within a complex and often confusing health care system.

At Elder Care Guides, our mission is to improve the lives of adults in San Diego County living with functional impairments by providing professional care management and in-home services that center on the goals of the individual, emphasize their strengths, and foster a lifelong sense of purpose. We believe that by cultivating every client’s unique sense of purpose, we help them develop a meaningful elderhood with a high quality of life, and that by identifying and reinforcing the strengths of our clients, we promote the development of healthy relationships of interdependence amongst the elder, their family, and other supports.
Founded in 2004 by local certified Care Managers Amy Abrams and Norman Hannay, Elder Care Guides is independently owned, and has been serving San Diego families and the professional community for over a decade. All of the members of our clinical team are members of the Aging Life Care Association.
